Physical Therapist / Occupational Therapist / Speech Language Pathologist

Assignment Duration

This is a 13-week contract position.

Schedule

Monday - Friday, 5 shifts per week, 8 hours per shift.

Job Description

We are seeking dedicated Physical Therapists, Occupational Therapists, or Speech Language Pathologists to provide high-quality home health care services. The ideal candidates will have experience with Homecare Homebase documentation and a passion for delivering exceptional care to patients in their homes.

Responsibilities

• Conduct average 4-6 patient visits per day.
• Complete evaluations, treatment plans, and therapy interventions as per discipline.
• Document patient care and progress using Homecare Homebase.
• Follow productivity metrics of 28-32 points per week.
• Participate in weekly case conferences and ongoing patient assessments.

Required Experience / Certifications / Licensure

• Experience in Home Health settings required; OASIS knowledge is a must.
• Valid state licensure relevant to the discipline (PT, OT, SLP).
• Homecare Homebase documentation experience preferred.
• Consideration may be given without HCHB experience for candidates willing to train for two weeks at a reduced rate.
