Certified Occupational Therapy AssistantJob Description Summary:The Certified Occupational Therapy Assistant provides occupational therapy servicesunder the direction of the Occupational Therapist and according to the physician's planof care.Essential Job Functions:A. Participates in the ongoing evaluation of patient's functional status (muscle function,endurance, visual coordination, written and verbal communication skills, self careability, work capacity, etc.) as delegated by the registered occupational therapist.Participates in the ongoing evaluation of the home environment for hazards orbarriers to more independent living as delegated by the Registered OccupationalTherapist.B. Participates in teaching new skills or retraining patients in once familiar dailyactivities that have been lost due to illness or injury, in accordance with organizationpolicy.C. Maintains appropriate clinical records, clinical notes, and reports to the RegisteredOccupational Therapist any changes in the patient's condition.D. Submits clinical documentation in accordance with Organization policy.E. Follows treatment program and goals for improved patient function as established bythe Registered Occupational Therapist. Documents patient's response to treatmentplan and progress toward established goals.F. Maintains contact/communication with other personnel involved in the patient's careto promote coordinated, efficient care. Documents such communication inaccordance with Organization policy.G. Attends and contributes to in-services, case conferences, and other meetings asrequired by Organization policy to ensure coordinated and comprehensive plans ofcare for the patients of the Organization.Qualifications:OTA LicenseDriver's License/Auto InsuranceCPR CertificationMinimum 1-year previous home health experiencePrevious experience using Home Care Home BaseFlexible schedule / multiple work locations
