A Day in the Life of a Physical Therapist As a Physical Therapist in home health care, you play a vital role in helping patients regain mobility, strength, and independence. Your responsibilities include: Conducting initial and ongoing assessments to evaluate patient function Collaborating with physicians to develop or revise individualized plans of care Preparing clinical and progress notes in accordance with regulatory standards Providing therapeutic treatments tailored to each patient's needs Consulting with families and agency staff to support patient goals Continuously evaluating treatment effectiveness and updating plans as needed Coordinating services with other home health team members Supervising physical therapy assistants and home health aides when appropriate Assessing equipment needs to ensure optimal patient outcomes Following CMS documentation and billing requirements, including functional reassessments Practicing infection control and universal precautions Reporting suspected abuse according to state and federal guidelines Assisting with new‑hire orientation and participating in training activities You take pride in delivering high‑quality care that directly improves patients' lives. Qualifications Successful completion of an accredited physical therapy education program Current Oklahoma PT license in good standing At least 1 year of physical therapy experience CPR certification (or ability to obtain within 60 days) Ability to meet the physical demands of the job We're looking for someone who communicates clearly, stays organized, manages time well, and maintains a positive, professional demeanor.
