On Call: Dependent on Locums/Part Time Providers Number

Medical Hematology/Oncology coverage for 2 OP at secondary clinics (no transplants), and 3 OP days per week at primary clinic. IP coverage of both sites; APP will cover for provider.

Number of advanced Practice Providers: 3

Supervision of APP expected: Yes

Supervisor name: Primary Locums Physician

Nursing support/Physician Nurse Ratio: Non-nursing roles

Ancillary Staff per provider: 0.5 (scheduler, biller, CNA)

Number of Exam rooms per provider: 2

Medical Imaging: Yes

Lab & Pharmacy: Yes

Services/Procedures offered in the practice: Patient assessments, new treatment plans, ongoing treatment plans, changes in therapy, referral for ancillary services, end of life care, chemotherapy management, pain management... (radiation and surgical oncology management provided by existing providers with strong relationships)

Clinic Provider daily volume: 10-30

Submission Requirements:

Board Certified - REQUIRED (Board Certified in Medical Oncology is required. Board Certified in Hematology is preferred but not required)

Fellowship trained in Medical Oncology/Hematology - REQUIRED

IA License - REQUIRED
