Licensed Practical Nurse - LTC

LPNWe are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. The ideal candidate will play a vital role in providing high-quality care to patients with diverse healthcare needs.Key ResponsibilitiesProvide direct patient care under the supervision of registered nurses and physicians.Monitor and record patient vital signs, as well as assist with daily living activities.Administer medications and treatments as prescribed, ensuring adherence to safety protocols.Collaborate with healthcare team members to develop and implement individualized care plans.Maintain accurate and up-to-date patient records using Electronic Medical Records (EMR) system (PCC).Educate patients and their families on health maintenance and disease prevention strategies.QualificationsCurrent licensure as a Licensed Practical Nurse (LPN).Certification in IV therapy is required.Strong interpersonal and communication skills.Ability to work effectively in a fast-paced and changing environment.Experience in long-term care or similar healthcare facilities is preferred.Work ScheduleEvery other weekend commitment is required.Holidays will also be scheduled on an alternate basis.Shifts available in both 8-hour and 12-hour formats.OrientationLocation-specific orientation will be provided, requiring 1-2 shifts to familiarize with the facility protocols and procedures.Patient PopulationThe candidate will primarily care for individuals with conditions such as dementia, diabetes, hypertension, chronic kidney disease, and those requiring assistance with activities of daily living (ADLs).Important NotesLocal rates apply within 80 miles or when using our housing options.There is no charge differential at our facilities.Fees will be incurred for backouts and quits with less than 14 days notice.If you are a proactive LPN with a passion for patient care, we encourage you to apply and join our team in making a difference in the lives of our patients.
