Overview Our Mobile Geriatric Psychiatry Team is seeking a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide high‑quality psychiatric services directly to seniors in private homes and senior living communities. What Makes This Role Unique Deep expertise in geriatric psychiatry, including dementia‑related behaviors, depression, anxiety, and complex neurocognitive conditions. Personalized psychiatric care delivered in home and senior living settings, fostering stronger relationships and informed decisions. Practice independently while supported by a dedicated medical assistant and medical scribe. Quality outcomes, continuity, and provider satisfaction are core; growth is supported without sacrificing care standards. Your Responsibilities Include Conduct comprehensive psychiatric evaluations for geriatric patients in home and facility‑based settings. Diagnose and manage mood disorders, anxiety disorders, dementia‑related behavioral symptoms, and other psychiatric conditions. Prescribe and manage psychotropic medications with careful consideration of comorbidities and geriatric physiology. Provide crisis intervention, behavioral guidance, and family education when clinically indicated. Collaborate with primary care providers, therapists, and facility care teams to ensure coordinated, whole‑person care. Build trusted relationships with patients, caregivers, families, and community partners. Maintain consistent, accurate clinical documentation and follow‑through.
