Work Arrangement: On-site as needed. This is not a patient-facing position.

Schedule: Friday through Tuesday, 4:00 PM–1:00 AM. This schedule includes both Saturday and Sunday. Candidates must be available to work the required evening and weekend shifts.

Position Summary:

The Pharmacist – Evening Shift is responsible for dispensing prescriptions, coordinating clinical services for specialty medications, and ensuring the accuracy and safety of prescription orders. This role involves reviewing patient profiles, evaluating potential drug interactions and sensitivities, compounding medications, resolving prescription issues, and coordinating with physicians, patients, and other healthcare professionals.

The pharmacist will also handle prescription-related questions, resolve orders through the External Adjudication (EA) queue, support therapeutic interventions, and maintain compliance with quality assurance procedures, controlled substance security requirements, and hazardous pharmaceutical waste disposal standards.

Key Responsibilities:

• Prescription Review: Review patient profiles, medication histories, drug interactions, sensitivities, and drug usage databases to assess prescription appropriateness and patient safety.
• Prescription Processing: Enter prescriptions accurately into the pharmacy computer system and process orders in accordance with established procedures.
• Medication Dispensing: Dispense prescribed medications and perform final quality checks to verify prescription accuracy and appropriate dosage.
• Prescription Translation: Interpret and translate prescription information as needed to support accurate processing and dispensing.
• Medication Compounding: Compound pharmaceutical products in accordance with applicable procedures and quality standards.
• Specialty Pharmacy Services: Coordinate clinical services for specialty medications serviced through the branch location.
• Clinical Assessment: Determine whether prescriptions require clarification from the prescribing physician or referral for therapeutic intervention or follow-up.
• Physician Communication: Contact physicians to resolve prescription questions, clarify orders, and address medication-related concerns.
• Patient Support: Respond to incoming patient questions and concerns regarding prescription medications and provide appropriate assistance.
• Customer Care Coordination: Communicate with patients and healthcare professionals by telephone or mail to support prescription processing and continuity of care.
• Therapeutic Intervention: Assist with therapeutic intervention activities and physician follow-up calls as assigned.
• External Adjudication: Resolve prescription orders through the External Adjudication (EA) queue.
• Quality Assurance: Follow established quality assurance procedures and pharmacy standards to maintain safe and accurate dispensing practices.
• Controlled Substances: Maintain compliance with established security procedures for controlled substances.
• Hazardous Waste Disposal: Follow applicable procedures for the proper disposal of hazardous pharmaceutical waste.
• Team Collaboration: Work with other pharmacy team members to ensure prescription accuracy, operational efficiency, and appropriate coordination of patient care.
• Interpersonal Communication: Maintain professional, effective communication with patients, physicians, and other healthcare professionals.

Required Qualifications:

• Education: High school diploma or GED required. A verifiable high school diploma or GED is required.
• Pharmacy Degree: Bachelor's degree in Pharmacy or Doctor of Pharmacy (Pharm.D.) required.
• Pharmacist Experience: At least two years of experience working as a pharmacist required.
• Clinical IV/Infusion Experience: Minimum of one year of clinical IV/infusion experience required. This is a strict requirement; candidates without this experience will not be considered.
• Licensure: Must possess an appropriate, active state pharmacist license in good standing.
• Computer Skills: Basic computer skills are required, including the ability to enter prescriptions and navigate pharmacy systems.
• Clinical Knowledge: Must understand prescription processing, medication dispensing, drug interactions, patient sensitivities, and medication usage.
• Attention to Detail: Must demonstrate accuracy when reviewing patient profiles, entering prescriptions, compounding medications, and completing final dispensing checks.
