Home Health Physical Therapist PT Job Summary: PathWell Health, a trusted and community-focused home health care provider, is seeking a highly skilled and compassionate Physical Therapist to join our team. - Develop, implement, and adjust a conditioning/rehabilitation plan of care in partnership with patients, representatives, caregivers, and physicians
- Test and screen patients' physical strength to assist physicians in evaluating their level of function
- Educate and instruct patients, family members, or representatives about exercise programs and therapeutic procedures
- Accurately document observations, interventions, and evaluations pertains to patient care management and services provided
- Provide direct patient care as prescribed, including evaluation and treatment planning
- Review physicians' referrals and patients' medical records to determine diagnosis and physical therapy treatment required
- Graduation from an approved Physical Therapy curriculum
- Current and valid Connecticut Physical Therapist license
- Recent work experience in home health (preferred)
- Driver's License, Car Insurance, and Car Registration
- Strong clinical assessment, organizational, and communication skills
- Ability to handle multiple assignments simultaneously
- Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation
- Bilingual (Spanish) preferred
- Strong interpersonal skills and good judgment
- Ability to work well under pressure and stay calm and focused in emergent situations
- Be highly self-motivated and results-driven
- A desire for further education to keep up with the ever-changing medical advances

- While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands or fingers, handle or feel objects, tools, or controls. The employee is occasionally required to stand; walk; sit; and reach with hands and arms. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this position include close vision, distance vision, and the ability to adjust focus.
- The work environment is typically a patient's home, with occasional visits to medical facilities or our office.
- $90,000 - $120,000 ANNUALLY
- PART-TIME AND FULL-TIME
