Position Summary:

Our Tennessee region is seeking a Registered Dietitian to join our team in a skilled nursing setting. This role offers flexibility to support a variety of schedules, including full-time, part-time, or hybrid opportunities depending on candidate preference and community needs.

Core Responsibilities:

• Complete timely, accurate nutrition assessments, follow-ups, and documentation in accordance with CMS regulations and facility policies

• Develop and implement individualized nutrition care plans that align with physician orders and resident goals

• Monitor residents at nutritional risk, including weight changes, wounds, tube feeding, and altered intake

• Collaborate with nursing, therapy, food service team, and medical providers to ensure coordinated care

• Ensure diet orders are accurate, implemented correctly, and communicated effectively to the food service team

• For onsite employees conducting audits for food service department sanitation, accuracy of meal delivery and residents feedback of meals

• Collaborate with the interdisciplinary team on survey risk areas, regulatory deadlines, and facility level initiatives.

• Support survey readiness through compliant documentation and participation in audits as needed

• Provide education to residents, families, and staff as appropriate

• Participate in Quality Assurance and Performance Improvement (QAPI) activities

 

Qualifications:

·       Must be a Registered Dietitian with CDR (Commission on Dietetic Registration)

·       Must hold a state license in the state of which you are applying if indicated

• Must be willing to oversee multiple communities throughout the state of TN

·       1 Year experience preferred

This is a hybrid roll with onsite responsibilities
