We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at The Avenue at Lyndhurst, a post-hospital rehabilitation and skilled nursing care facility in Lyndhurst, Ohio. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our residents and patients. With a focus on comfort, dignity, and healing, our state-of-the-art facility is unique and supportive for individuals transitioning from hospital to home. Responsibilities: Provide skilled nursing care and support to residents and patients under the supervision of licensed nurses and physicians Administer medications, treatments, and therapies as prescribed by physicians and according to established protocols Monitor and maintain accurate records of patient progress, medication administration, and treatment plans Develop and implement individualized care plans to meet the unique needs of each resident or patient Participate in interdisciplinary team meetings to discuss patient care and develop effective treatment strategies Qualifications: Must be a Licensed Practical Nurse (LPN) currently licensed in the State of employment Genuine interest in geriatric nursing and long-term care Ability to make sound judgments and decisions Physically fit, mentally and emotionally stable Familiarity with facility policies and procedures Strong communication and interpersonal skills We are an Equal Opportunity Employer.
