What You'll Do

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans
• Deliver evidence-based outpatient physical therapy focused on improving strength, mobility, balance, endurance, and functional independence
• Treat patients recovering from orthopedic injuries, surgery, chronic conditions, and mobility limitations
• Provide gait training, balance training, therapeutic exercise, manual therapy, and patient education
• Collaborate with physicians, therapists, nurses, and other healthcare professionals to support optimal patient outcomes
• Document evaluations, progress notes, and discharge recommendations in accordance with organizational standards
• Participate in safe transitions of care and interdisciplinary discharge planning

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Tennessee Physical Therapist license or eligibility
• Outpatient rehabilitation experience preferred
• Strong communication, collaboration, and patient education skills

KEY RESPONSIBILITIES

• Demonstrates safe and effective treatment/ management of performance areas with all assigned patients

• Consistently communicates to PTA and other clinicians involved in the patient care
• Consistently communicates with other outside entities directly involved with patient outcomes
