Job Description You will be responsible for planning and coordinating admission to the facility. The admission coordinator seeks and/or receives inquires and documents information for potential residents to be admitted. Responsibilities

• Seek and/or receive inquires and documents for potential residents to be admitted
• Screens and evaluated each referral and determine whether the prospective resident meets criteria for transfer and admission
• Coordinates all communication received from physician
• Documentation of all admission and discharge activity
• Responsible for marketing efforts in the community

Qualifications

• Current knowledge of state and regulatory requirement related to admissions, discharge and transfer is demonstrated
• Current knowledge and understanding of payment systems, HMO, PPO, Indemnity, Worker’s Compensation, insurance (Medicare and Medicaid, etc)
• At least one (1) year of experience in the health care field, preferably long-term care
• Excellent interpersonal skills. Effective written and oral communication skills to communicate with health care professionals, potential residents, family members and the community in general.
