Job Title: Nurse Practitioner Location: Catonsville, Maryland Company: Carease Health - MD Job Summary Carease Health - MD, a nationwide preventative healthcare company, is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner, you will play a pivotal role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to minimize hospitalizations and elevate patient care standards.
