# Bloodless Care Coordinator (Part time) Jefferson AbingtonApply: Montgomery County, PA: Part time: Posted Today: REQ-0034153**Number of Positions In Requisition**1**Job Details**The Bloodless Medicine Care Coordinator coordinates a high-quality and consistent standard of interaction with patients and care team members within Jefferson Health/Abington. The liaison will serve as an advocate, educator, and facilitator for patients requiring blood management. Collaborates with care team to facilitate the needs of the patient. Serves as liaison and resource to patients, providers, families, and patient care colleagues.**Job Description****ESSENTIAL FUNCTIONS**• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.• Conducts daily patient rounds and chart review, generates bloodless program inpatient report daily, assists with data collection, compiles monthly activity, referral and statistical reports.• Serves as liaison and resource to patients, providers, families, and colleagues regarding blood management.• Assists patients in completion of advanced directives related to alternatives to blood utilization.
