ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES

Develops, strategizes, implements, and maintains program initiatives, assessments for evaluation and organizational standards of satisfaction, quality, and performance to meet PCMH practices. Ensures security and privacy of data in all FDIHB, Inc. associated systems. Lead PCMH transformation and ongoing compliance with accreditation standards. Collect and submit data to AHCCCS for American Indian Medical Home (AIMH) designation. Prepare and manage documentation for PCMH recognition/accreditation. Monitor adherence to policies, workflows, and care delivery standards. Support care coordination across providers, specialists, and community resources. Promote patient-centered care, including shared decision-making and self-management support. Track, analyze, and report clinical quality metrics. Lead quality improvement initiatives. Identify gaps in care and assist with corrective action plans. Facilitate interdisciplinary team meetings with stakeholders. Train staff on PCMH principles and workflows. Serve as a liaison between leadership, providers, nurses, and community resources. Utilize the Electronic Health Record (EHR) system to extract and analyze data. Ensure accurate documentation and data integrity. Performs other duties as assigned.

MANDATORY MINIMUM QUALIFICATIONS

Experience: Three (3) years of direct work experience in quality improvement in a health care setting, data analytics or care coordination in a Primary Care setting. Education: Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or closely related field. Must obtain PMCH certification (Joint Commission, AAAHC, or NCQA) within two (2) years from date of hire and maintain certification. NAVAJO/INDIAN PREFERENCE FDIHB and its facilities are located within the Navajo Nation and, in accordance with Navajo Nation law, has implemented a Navajo/Indian Preference in Employment Policy.
