About This Role The Nurse Navigator will serve as a primary contact for oncology patients, caregivers, and referring physicians, facilitating access to clinical and supportive services throughout the oncology care continuum. Highlights Location: Houston, TX Facility: Kelsey-Seybold Clinic Brand/Division: Optum Schedule: Full Time Pay: $60,200 - $107,400 annually What You'll Do Coordinate patient referrals to Hematology/Oncology. Schedule appointments within established guidelines. Collaborate with the multidisciplinary team for patient care. Maintain professional relationships with physicians and staff. Demonstrate a positive attitude aligned with clinic values. Ensure workspace safety and adhere to safety regulations. Comply with State, Federal, and local regulations and guidelines. Stay current on job-related knowledge and skills. Provide education and emotional support to patients and families. Assist in coordinating end-of-life care for patients and families.
