Surgery - General Physician Assistant

SCHEDULE

• Coverage Type: Inpatient
• Schedule Type: Hospital only
• Call Type: No Call
• Dates of Need: 3 days a week, 4 weeks a month are needed. Will be flexible on when they can pick up shifts during the week.
• Shift Times: 6:30am-4:30pm
• Lunch N/A
• Provider Min # Shifts Required: 12 days a month

Please ensure the bid reflects the below:

• Guaranteed Hours per Day: 8
• Hours Worked for OT to apply: 8 hours per day

OT must be preapproved by the medical director by end of shift.

• Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday

CREDENTIALING

• Current CV (must be uploaded to bid)
• License: CA Active
• DEA Required: Yes
• Boards: BC or truly BE only

PLEASE ENSURE AN UPDATED CV WITH ALL HOSPITAL AFFILIATIONS IS UPLOADED DURING PRESENTATION.

Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation.

o All HBS specialties require 2 years of current Hospital clinical activity in the specialty.
o Gaps greater than 3 months must be explained.
o Include current number of case logs upon presentation for surgical specialties.

ADDITIONAL DETAILS

Providers won't be in the OR.

Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):
NOT reimbursed:
o Mileage from provider's home to departing airport, as well as from airport to home upon return.
o Ride share (Lyft/Uber/etc.) and/or taxi from provider's home to departing airport, as well as from airport to home upon return.

WILL be reimbursed:
o Airport parking at provider's departing airport if provider drives themselves to the airport.
o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.
o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips.
