Registered Nurse – Home Health

M-F until shift is complete

Assignment Details

Guaranteed Hours: 40 Contract Weeks: 9

Requirements

• Active RN license
• Minimum of two years nursing experience
• One year experience in home health
• Oasis, HomeCare, HomeBase experience required
• Valid state driver's license and automobile liability insurance
• Able to drive an automobile in all types of weather conditions

Responsibilities

• Ensure individualized and appropriate client care plans are executed and reassessed as necessary.
• Complete accurate documentation for all patients transferred to inpatient facilities.
• Review and process OASIS assessments and documentation for accuracy.
• Manage referral intake and ensure assessment visits are scheduled and performed timely.
• Lead case conferences and document findings and decisions effectively.
• Communicate with physicians to obtain necessary service orders.
• Review and approve orders and follow up on any discrepancies.
• Document and ensure follow-up on vital sign alerts and wound score deviations.
• Participate in patient visits and on-call rotation as needed.
