To further this mission, Callen-Lorde promotes health education and wellness, and advocates for LGBTQ+ health issues.About the Role:The Doula Surgery Integrated Health Navigator will provide assistance to transgender and gender non-binary (TGNB) patients in navigating medical and supportive service systems related to recovery care.General responsibilities include:Perform assessments and develop a personalized care plan.Develop relationships with community organizations supporting TGNB communities.Conduct outreach activities to include community visits and consults.Follow up appointments.Assist in the development of health education materials focused on the health needs and concerns of TGNB communities.Work Schedule/Benefits:Callen-Lorde is open Monday - Saturday.
