Requirements:

• 2 years of experience as a PT
• Qualified applicants MUST have at least 2 years of experience in Home Health
• Qualified applicants MUST have Valid Physical Therapist license
• Valid Physical Therapist license
• Be willing to obtain WA licensure

Responsibilities: As a healthcare professional in a clinical setting, you’ll help patients move through life with confidence and care.

• Deliver one-on-one, high-touch PT care to patients in their homes, providing individualized Physical Therapy services.

• Conduct skilled evaluations/assessments, develop and manage individualized care plans, and track patient progress for improved outcomes.

• Collaborate with families and interdisciplinary providers to coordinate home health services and support patient-centered care using common home health and PT industry practices.
