Position Physical Therapist – Inpatient and Outpatient Specialist (Minneapolis, MN) Responsibilities Assess patients across both inpatient and outpatient settings and craft personalized treatment plans tailored to individual needs Deliver physical therapy treatments focused on enhancing mobility, strength, balance, and overall functional independence Work collaboratively with nurses, physicians, case managers, and a team of rehabilitation professionals to achieve the best clinical outcomes Educate patients and their families regarding therapy goals, discharge procedures, and home care exercises Diligently document evaluations, treatment notes, and patient progress in alignment with institutional and regulatory standards Engage in care team meetings and interdisciplinary discussions to ensure holistic and coordinated care for every patient As a Physical Therapist, you will conduct thorough evaluations, implement targeted treatment protocols, modify care paths as patients progress towards healing, and play an integral role in ensuring smooth discharge planning and safe transitions back into everyday life.
