Rehabilitation Therapy - Physical Therapist - I/II

PT in Tuba City, Arizona. Schedule: 8:00AM - 5:00PM x 5. Term: 12-week assignment with the potential for extension. Pay Rate: $62 - $67/hr.

Responsibilities for a PT:

• Provide quality physical therapy services to a predominately Native American population for the treatment or prevention of physical disability or disease
• Manage and implement the Physical Therapy program
• Provide physical therapy evaluation and treatment of inpatients during regular work week and weekend rotation
• Provide physical therapy evaluation and treatment of outpatients, serving patients ranging in age from birth through geriatric
• Deliver specialty services including wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, and assistive technology/adaptive equipment evaluation and treatment
• Support the Durable Medical Equipment Prosthetic Orthotic Supply Program
• Assume direction and development of the physical therapy section
• Oversee continuing quality improvement, risk management, and safety initiatives
• Develop and maintain physical therapy policies and procedures
• Prepare for and participate in Joint Commission activities
• Provide staff education, lead staff meetings, and conduct in-service training
• Complete reporting responsibilities as required
• Fulfill responsibilities listed on the Medical Staff Credentialing Core Privileges form
• Maintain a valid, current, full, and unrestricted physical therapy license
• Maintain a current Basic Life Support (BLS) certification by the American Heart Association
• Meet and maintain requirements for credentialing on the Medical Staff
• Submit all information required for Medical Staff credentialing
