Position:Lead Palliative Care PhysicianMonogram Health is seeking a full-time physician specializing in palliative care to join our team amid continued nationwide growth. This is an inmarket, fieldbased position requiring residence within one of our approved territories. As an inmarket specialist, this physician will care for seriously ill patients in their community, including individuals with CKD/ESRD, COPD, and CHF. The Lead Palliative Care Physician will work closely with the Market Physician Executive (MPE) and primarily deliver care in the home.Your ImpactMonogram Health physicians are quality-driven physicians who apply their clinical expertise in a managed care, population health context. Final compensation will be determined based on geographic location and job-related factors such as experience, skills, education, certifications, and internal compensation considerations. While a larger range is posted, most offers will typically fall within a narrower band based on these factors.In addition to base salary, this role is eligible for additional compensation, including bonus opportunities and a comprehensive benefits package.Roles & ResponsibilitiesCollaborate with MPE and the care team to proactively identify patients who qualify for palliative care services and build a patient panelDeveloping and presenting metrics to executive leadership, as requestedResponsible for the outcomes for patient panel, driving both predetermined clinical and financial Key Performance Indicators (KPIs) through interventions outlined belowTreat panel of patients in-home to drive healthcare outcomesEvaluate and treat symptoms that may interfere with quality of life, like pain, pruritis, depression, anxiety, insomnia, etc.Refer members to appropriate specialists to drive clinical care (Monogram-based specialists or locally-aligned specialists)Conduct goals of care and family meetings as needed and document patient/family wishes in state AD/POLST formsSupport patients utilizing our IDT, including Palliative Care ChaplainAssist with hospice transitions as appropriateDevelop key relationships with local providers (PCP, nephrologists, specialists) to improve market alignmentParticipate in monthly meetings with RMD/CMO in areas of KPI and expectations around KPIsEducate and support team around palliative care, hospice care, advance directives, symptom assessment, and communicationPosition RequirementsMust be willing and able to obtain hospital privileges at required facilities.Desire to practice in a forward-looking renal care environment focused on population health managementDemonstrated experience applying evidence-based clinical criteriaComfort and skill in goals of care discussions and advance care planningExperience with high-need Medicare Advantage and managed Medicaid populations, as well as commercial populationsExperience integrating acute and behavioral health care and appreciation of social determinants of health and the value of social workersExpertise in managing medication panels for complex patientsCurrent state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare.Willingness to become licensed in multiple states as company and patient needs requireDoctorate Degree in MedicineBoard Certified in Hospice and Palliative MedicineMust have reliable transportationBenefitsComprehensive Benefits - Medical, dental, and vision insurance, employee assistance program, employer-paid and voluntary life insurance, disability insurance, plus health and flexible spending accountsFinancial & Retirement Support– Competitive compensation, 401k with employer match, and financial wellness resourcesTime Off & Leave– 20 PTO days, 7 paid holidays, and 1 floating day to make your own, and paid parental leaveWellness & Growth – Work life assistance resources, physical wellness perks, mental health support, employee referral program, and BenefitHub for employee discountsAbout Monogram HealthMonogram Health is a leading multispecialty provider of in-home, evidence-based care for the most complex of patients who have multiple chronic conditions.
