Location: Stroudsburg Post Acute Nursing and Rehab

About the Role

We are seeking a passionate and dedicated PRN Physical Therapist to join our dynamic rehabilitation team in our skilled nursing facility in Stroudsburg, Pennsylvania. If you love helping seniors regain their mobility, independence, and quality of life, this is the perfect opportunity for you!

What You’ll Do

•  Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions

•  Develop personalized treatment plans to improve strength, balance, gait, and functional mobility

•  Collaborate closely with physicians, nurses, occupational therapists, and speech therapists

•  Document patient progress accurately and in a timely manner using our electronic medical records system

•  Participate in care plan meetings and family conferences

•  Provide compassionate, patient-centered care in a supportive team environment

•  Supervise Physical Therapist Assistants (PTAs) as needed

•  Report directly to the Director of Rehabilitation

What We’re Looking For

•  Active Physical Therapist license in the state of Pennsylvania (or eligible to obtain)

•  New grads welcome! We provide excellent mentorship and training

•  Previous experience in SNF, long-term care, or geriatrics is a plus

•  Strong communication and interpersonal skills

•  Ability to work collaboratively as part of an interdisciplinary team

•  Compassionate, positive attitude with a genuine desire to make a difference in residents’ lives
