Registered Nurse Transplant Coordinator

Las Palmas Medical Center in El Paso, Texas, is seeking a compassionate and experienced Registered Nurse Transplant Coordinator to join our Healthcare & Medical Services team. As a key member of our transplant and acute care services, you will coordinate the full continuum of care for transplant candidates, recipients, and living donors while working closely with our ER and inpatient teams. This role blends advanced clinical practice, patient advocacy, and complex care coordination to support some of our most vulnerable patients and their families.

The Registered Nurse Transplant Coordinator serves as the primary clinical liaison for transplant patients, guiding them through evaluation, listing, transplantation, and long-term follow-up. You will conduct comprehensive nursing assessments, collaborate with transplant surgeons, physicians, social workers, and allied health professionals, and develop individualized care plans tailored to each patient’s clinical and psychosocial needs. You will educate patients and families on the transplant process, associated risks and benefits, immunosuppressive medications, lifestyle modifications, and early recognition of complications.

In collaboration with the ER team, you will help identify and support potential donors and manage urgent and emergent situations involving transplant patients who present with acute complications or rejection. Drawing on strong ER and acute care skills, you will perform rapid assessments, monitor hemodynamic status, and coordinate timely interventions and transfers. You will also facilitate diagnostic testing, lab work, imaging, and specialty consultations required during pre- and post-transplant phases.

This position requires meticulous attention to detail and a deep understanding of transplant protocols, organ allocation and procurement processes, and regulatory requirements. You will ensure accurate and timely documentation in the electronic medical record and transplant databases, maintain compliance with standards set by regulatory bodies such as UNOS, CMS, and The Joint Commission, and participate in multidisciplinary selection conferences presenting clinical findings and recommendations on transplant candidacy.

As part of Las Palmas Medical Center’s collaborative culture, you will partner with physicians, nurses, pharmacists, case managers, and social workers to deliver coordinated, compassionate care that addresses the diverse health needs of the El Paso community. You will serve as a clinical resource to nursing staff, assist with the development and implementation of transplant-related policies and procedures, and contribute to ongoing quality improvement and patient safety initiatives. You may also participate in community outreach and educational activities that promote organ donation awareness and understanding of transplant services.

The ideal candidate is a Registered Nurse with strong clinical judgment, excellent communication skills, and the ability to remain calm and effective in intense situations. You should be comfortable managing complex cases, prioritizing multiple demands, and advocating for patients and families during highly stressful and emotional experiences.
