*Position Summary:*

The Nurse Practitioner (APRN) provides comprehensive primary care services including preventative care, acute visits, and chronic disease management. The ideal candidate demonstrates clinical excellence, strong patient communication skills, and a collaborative spirit aligned with our mission and values.

*Qualifications:*

* Masters Degree in Nursing (Required)
* Active Arizona APRN License
* Minimum 1 year experience as an APRN
* Prescriptive authority & DEA registration
* Commitment to ethical, patient-centered care
* Alignment with the mission and core values of Hometown Healthcare

*Responsibilities:*

* Perform comprehensive physical examinations
* Review and update patient medical records
* Order and interpret diagnostic testing (labs, imaging, etc.)
* Diagnose and manage acute and chronic conditions
* Develop individualized treatment plans
* Prescribe medications in accordance with evidence-based practice
* Educate patients and families on prevention and wellness
* Promote preventative medicine and healthy lifestyle practices
* Maintain accurate documentation and scheduling
* Participate in continuing education and professional growth
