Occupational Therapist

Evaluate residents’ functional abilities, including activities of daily living (ADLs), mobility, cognition, and psychosocial factors.

Develop and implement individualized occupational therapy treatment plans based on assessments and physician orders.

Deliver direct occupational therapy interventions to improve residents’ functional independence and quality of life.

Monitor, document, and adjust treatment plans according to residents’ progress and needs.

Collaborate with interdisciplinary team members, residents, and families to achieve optimal rehabilitation outcomes.

Educate residents, families, and caregivers on safety and independence strategies.

Recommend and instruct on the use of adaptive equipment and assistive devices.

Supervise Certified Occupational Therapy Assistants (COTAs) and therapy aides as needed.

Ensure timely, accurate, and compliant documentation in electronic medical records.

Maintain licensure and fulfill continuing education requirements as per state regulations.

Support quality improvement initiatives and adhere to CMS, state, and facility policies and guidelines.

Work in a skilled nursing facility (SNF) or long-term care setting, requiring physical activity and teamwork in a fast-paced environment.
