LPN Case Manager II - PASSPORT HCBS

Direction-Home-of-Eastern-Ohi is seeking a Case Manager in Austintown Township, Ohio, responsible for managing PASSPORT program services. The Case Manager will coordinate services to support consumers safely in their homes. The ideal candidate must hold a Licensed Practical Nurse certification, possess strong communication skills, and be willing to travel within the four-county service area.
