Licensed Vocational Medication Nurse (LVN) - New Grads Day Shift Location: Santa Monica, CA Role: Full-time, Day Shift Salary Range: $32.00 – $35.00 hourly About CLARE|MATRIX CLARE|MATRIX is a nonprofit organization that provides effective and compassionate treatment, recovery, and prevention services for alcoholism and substance use to individuals, families, and the community. Founded in 1970, the organization maintains multiple facilities across Southern California. Summary Under the supervision of a licensed healthcare professional, the LVN works closely with the treatment team to provide nursing care in a 24/7 residential and recovery facility, ensuring clients are safely monitored throughout their stay. Essential Duties and Responsibilities Patient Care Provide nursing care, including monitoring vital signs, assessing withdrawal symptoms, and reporting changes in patient condition. Support oversight of participant day‑to‑day operations and services. Conduct timely and comprehensive bed checks according to schedule and program guidelines. Assist management and treatment teams with client supervision and service delivery. Testing and Documentation Ensure regular drug screens are conducted as directed, logging tests appropriately. Complete incident reports as required. Assist in maintaining program records and files. Maintain accurate and timely documentation of patient care in electronic health records (EHR). Provide clear shift reports to ensure continuity of care. Safety and Compliance Adhere to all state and federal regulations, including California Board of Vocational Nursing standards, and prioritize patient safety and dignity. Follow all safety rules and regulations. Maintain HIPAA compliance, securing participant information and safeguarding medical records. Maintain professional boundaries and ensure a safe, ethical environment. Uphold health and safety regulations to CLARE|MATRIX standards, OSHA, CAL‑OSHA, and all regulatory requirements. Medication Administration Monitor medication stock levels, inventory, and storage, reporting low stock or discrepancies. Prepare, administer, and document all medications per physician orders and facility policies. Monitor patients for potential side effects or adverse reactions. Educate patients on medication purpose, side effects, and proper use. Billing for Medication Monitoring Complete notes in real time or by shift end to expedite billing. Submit urine analysis results in MyEvovle to support billing. Ensure health, physicals, and TB test data are reflected for timely and accurate billing. Teamwork and Collaboration Collaborate with nurses, therapists, and other providers to communicate participant changes, needs, updates, and progress. Participate in staff meetings, one‑on‑one meetings with supervisors, and other required meetings. Maintain knowledge and application of CLARE|MATRIX mission, vision, and philosophy. Engage in ongoing training, team meetings, and certifications to maintain eligibility. Qualifications Graduate of an accredited LVN program. Current and unrestricted California LVN license. Preferred: 1‑2 years of experience in a residential or substance abuse treatment setting. Knowledge, Skills, Competencies, and Experience Experience in residential or recovery center settings. Knowledge of residential treatment protocols and withdrawal management. Strong assessment and clinical skills with ability to respond rapidly to changes in patient condition. Excellent communication, organizational, and time‑management skills. Effective verbal and written communication and interpersonal skills. Organized and detail‑oriented. Self‑motivated and able to follow directions accurately. Ability to thrive in a fast‑paced environment. Physical Demands Performing responsibilities requires speaking and hearing, sitting, using hands and fingers, standing, walking, reaching, stooping, kneeling, crouching, and lifting up to 25 pounds. Vision required: close vision. Work Environment The position is located at CLARE|MATRIX facilities in Santa Monica, California. The environment is fast‑paced, dynamic, and may involve exposure to participants with challenging behaviors, including occasional aggression.
