• Pay Rate: $60.00-$65.00 / Hour (USD)
• Shift / Schedule: Day (5x8 Hours), 8:00 AM - 4:30 PM
• Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
• Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
• Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

Job Summary

This is a Home Health Physical Therapist travel/contract opportunity serving East Portland, OR and surrounding areas. The role involves providing skilled home health PT services in a 1:1 care model, visiting up to 6 patients per day with a standard productivity expectation of 30 points. The system does a good job grouping clients, minimizing drive and windshield time.

Key Responsibilities

• Provide skilled home health physical therapy services to patients in the East Portland, OR and surrounding areas
• Visit up to 6 patients per day in a 1:1 care ratio
• Meet a standard productivity expectation of 30 productivity points
• Complete clinical documentation using EPIC Remote Client for Home Health (on laptop), Rover (phone app), and Oasis
• Travel independently to patient homes; occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day

Required Skills

• Oasis experience required
• BCLS certification required
• Own reliable transportation required

Education Requirements

• PT License required
• BCLS certification required

Required Experience

• Minimum 1 year of skilled Home Health experience required

Work Environment / Physical Requirements

• Home health setting - travel to patient residences throughout East Portland, OR and surrounding areas
• Must have own transportation; occasional maximum drive radius of approximately 60 miles (60-90 minute total drive time) per day
• 8-hour day shifts (5x8); this facility cannot accommodate 4x10 or 3x12 schedules
• No on-call required
• No weekend requirement
• No floating between units required
• Block schedule not available
• Business casual attire; no jeans; scrubs allowed but not required (no specific color requirement)
• Charting via EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis
• Parking: Free
• Mileage Reimbursement: IRS Standard rate
