DHH Regional Care Coordinator

We are hiring for: DHH Regional Care Coordinator Type: Regular If you are a positive and personable individual looking for a satisfying and fun opportunity to make a real difference in the lives of people with intellectual, developmental disabilities, and people facing mental health, and substance use challenges, join our team at RHA Health Services! This position provides case management services to the Deaf and Hard of Hearing on a regional level. Relies on experience, knowledge, and education with limited direction to provide quality services to the Deaf and Hard of Hearing populations. Must have a Master’s Degree in a related field and one year of supervised, post‑graduated experience serving persons with mental health or substance abuse, or a Bachelor’s Degree in a Human Service field with two years of supervised, post‑graduated experience serving persons with mental health or substance abuse. Must have knowledge and experience with the age and population served. Be fluent in sign language, as determined by a Sign Language Proficiency Interview (SLPI) score of Advanced Plus or better. Responsibilities Provides on‑call/first responder services as scheduled at the local office. Plans and facilitates consumer transitions to other services and/or local agencies in an efficient manner. Maintains regular contact with the persons served, their treatment teams, guardians and significant others. Assures consistent provision of quality services and assesses the persons served to determine their needs, providing linkages/referrals to resources to help them achieve personal goals. Represents RHA at local management entities and other necessary meetings related to the care of the persons being served. All other duties as assigned. Education and Experience Master’s Degree in a related field with one year of supervised, post‑graduated experience serving persons with mental health or substance abuse, OR Bachelor’s Degree in a Human Service field with two years of supervised, post‑graduated experience, OR Bachelor’s Degree in another field with four years of supervised, post‑graduated experience serving persons with mental health or substance abuse. Must have knowledge and experience with the age and population served. Fluent in sign language, SLPI score Advanced Plus or better. Pre‑Employment Screening Complete criminal background check. Name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities). Drug testing. Education verification and other credentialing based on position requirements. Proof of employment history or references (if required). Positions that require driving: proof of driver’s license, driver’s insurance, and vehicle if required for transportation of individuals.
