Make a real difference in your community.DeKalb County Treatment Courts are seeking a compassionate, driven Opioid Remediation Case Manager to support individuals navigating recovery from Opioid Use Disorder (OUD) and co-occurring Substance Use Disorder or Mental Health conditions. In this role, you'll be a critical link between Treatment Court participants and the resources they need to rebuild their lives - connecting clients to healthcare, housing, counseling, and community support while helping them stay on track with their treatment and recovery plans.If you're passionate about second chances, skilled at building trust, and want a career where your work directly changes lives, we want to hear from you.What You'll DoAssess participant needs and build personalized care plansConnect clients to housing, healthcare, financial assistance, and other community resourcesCollaborate with healthcare providers, counselors, probation, and the Treatment Courts teamMonitor client progress and adherence to case plansProvide crisis intervention and support during emergenciesEducate clients and families on available servicesAttend court sessions, staffings, and meetings as part of the treatment teamWhat You BringExperience in social work, case management, or a related field preferredStrong written and verbal communication skillsSound judgment and problem-solving abilityCultural sensitivity and the ability to connect with a diverse caseloadWorking knowledge of community social service agenciesStrong organizational and time-management skillsProficiency in Word and ExcelA valid driver's license (required for client transport)Bilingual English/Spanish skills a plusGood to KnowThis position reports to the Treatment Courts Supervisor and DirectorEmployment is contingent upon a clear criminal background check and a TB screening testSome local travel required using a Treatment Courts vehicleThis position is grant-funded through 6/30/27, with renewal anticipated annually
