Certifications: Registration by the Commission on Dietetic Registration as a Registered Dietitian

Must-Have: Malnutrition Diagnosis, Critical illness experience, Diabetes Medication knowledge, Certified Diabetes Care and Education Specialist

Description: Monitors patients for and provides education on food/drug interactions. Records food and drug interactions in the education section. Consults with physicians regarding appropriate diet modification. Screens and assesses patients accurately and timely based on levels of care. Determines patients' nutritional needs and status and provides follow-up care. Interviews patients and/or their families to obtain history of food habits and preferences. Interprets pertinent laboratory, physical, and social data in the medical record as it applies to nutrition care. Provides nutrition education and records the patient's diet instructions. Documents the patient's understanding of the diet in the education section. Modifies patient menus as needed. Weekends and holidays are required, mostly 1 or 2 weekends per month. Interviews may be conducted via web or in person. Additional credentials such as Certified Nutrition Support Clinician are highly desired. Must comply with regulations for newly hired Dietitians assigned to Long-Term Care facilities.
