Your Role in Our Mission

• Conducting thorough patient evaluations, ordering necessary diagnostics, and developing personalized care strategies
• Diagnosing and managing acute and chronic conditions in collaboration with physicians or autonomously (depending on credentialing)
• Empowering patients and families through education on prevention, wellness, and chronic illness management
• Prescribing appropriate medications and therapies based on established guidelines
• Involvement in chronic care management and patient follow-ups
• Collaborating with multidisciplinary teams to guarantee comprehensive, holistic patient care
• Maintaining accurate and detailed records in electronic medical records (EMR)

Qualifications You Bring

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Graduation from an accredited Nurse Practitioner program

Essential Certifications

• Active Nurse Practitioner licensure in New York
• National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or equivalent specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to secure these within 90 days)
