Role Description This part-time, on-site Social Worker role is based in Riverside, CA. The Social Worker will provide psychosocial assessments, counseling, and emotional support to patients and families, helping them navigate the physical, emotional, and practical aspects of end-of-life care. Daily tasks include coordinating care plans with the interdisciplinary hospice team, facilitating family meetings, and connecting patients and caregivers with community resources and support services. The Social Worker will also assist with advance care planning, crisis intervention, and advocacy, ensuring that care is aligned with patient and family goals and cultural values. Documentation of assessments, interventions, and progress in accordance with regulatory and organizational standards is a key responsibility. Qualifications Possess strong clinical social work skills, including psychosocial assessment, counseling, and crisis intervention. Demonstrate ability to collaborate effectively with interdisciplinary teams, including nurses, physicians, chaplains, and other care professionals. Show proficiency in communication skills, including active listening, clear documentation, and sensitive conversations about end-of-life and bereavement. Exhibit knowledge of community resources, social services, and support organizations relevant to pediatric and adult hospice patients. Have experience or training in hospice, palliative care, or related healthcare settings; prior work with diverse populations is beneficial. Hold a Master’s or Bachelor’s degree in Social Work from an accredited program; licensure or eligibility for licensure in California preferred. Demonstrate cultural humility, respect for individual preferences and beliefs, and commitment to non-discriminatory, patient-centered care. Maintain strong organizational and time-management skills to handle multiple cases and documentation requirements in a part-time role. #J-18808-Ljbffr
