Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home health experience required.

Assignment Details

• Shift schedule: M-F with some weekends if needed.
• On-call not required.
• Territory: Lakeland area.
• Expected to go into the office 1x per week for a case conference.
• Orientation provided on the first day.
