LOCATION: Remote - must live in or near Vaya's Western NC region. Incumbent in this role must maintain residency in North Carolina or within 40 miles from the NC border. This is a home-based position that requires community travel . GENERAL STATEMENT OF JOB The Transitions to Community Living (TCL) Community Liaison's primary objectives are to provide education to members/recipients and entities stakeholders about TCL, educating providers on how to identify and report barriers, aid in eliminating barriers, provide and create training opportunities and how to access TCL and via the RSVP (Referral Screening Verification Program) process. This position works with various community stakeholders such as the individual, family, LRPs, community/state hospitals, providers, shelters, and adult care homes. This position will work closely and collaboratively with members of the TCL Team in order to meet the established time frames with requests for required documentation as needed through the process. This position ensures that the specific functions related to the Department of Justice and RSVP are followed. This is a mobile position that will require working in various locations throughout and beyond Vaya Health's catchment area. ESSENTIAL JOB FUNCTIONS Community Education/Liaison Responsibilities: A high level of diplomacy and discretion is required to effectively create buy in, negotiate, and resolve concerns or issues, this requires exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts. Problem solving, and conflict resolution skills are essential. Demonstrated effective leadership presence is required, with demonstrated confidence in dealing with unknown and complex issues. Awareness of and sensitivity to the complexities of stakeholder and participant concerns. Educate individuals members/recipients and referral entities (hospitals, providers, FCH, ACH, ALF and other key stakeholders) about the TCL eligibility requirements and the RSVP process. Educate and provide training opportunities to internal and external stakeholders regarding TCL program process and how to identify and report barriers. Cascade information to applicable departments within Vaya Health (Vaya) to assist with eliminating TCL barriers Coordinates with referral entities to address and assess the needs of the members/recipients, educate on appropriate services and levels of care as needed. Identifies gaps in services and intervenes to ensure that the individual receives appropriate care, can refer to MHSU Care Management, if appropriate. Ensures that services for the individual are identified and coordinated across the LME/MCO's system and with other systems, including primary care. Provides education and assistance in collaboration with local hospitals (and other stakeholders) as appropriate to RSVP. While providing outreach and education in the community, the Liaison may also provide potential members/recipients with information on TCL process. Educate facilities stakeholders when members/recipients do not meet TCL eligibility and provide additional community resources when inquiries are received. Other specific functions as they relate to Diversion, In-Reach, Transition Process and Post-Transition responsibilities as indicated from DMA/DMH. Performs related tasks as required. Collaboration : Collaborates with other members of TCL team and the community to identify areas of need and provides outreach and education in these areas. Serves as a collaborative partner in identifying system barriers. Attends monthly Vaya Local Barriers Committee as a "voting" member to discuss, collaborate and resolve TCL barriers. Sets up, manages, and facilitates meetings with community stakeholders as appropriate to TCL and RSVP process. Facilitates and coordinates the quarterly ACT/CST Collaborative Meeting with assistance across Vaya departments including Quality Management and Provider Network The TCL Community Liaison will work with consumers members/recipients and providers in multiple communities both within and outside the Vaya catchment Works in partnership with other LME/MCO departments to address identified needs within the catchment area. Collects data from concerning TCL via staff report of concerns and collaborates within teams to find resolution of needs. Documentation: The TCL Community Liaison is responsible for documenting in an accurate manner that is clear and concise. This documentation will serve to inform the local organization, state, and federal government. All contacts and interventions with members/recipients will be documented in the consumer's administrative health record if related to a specific individual need. The TCL Community Liaison is anticipated to support Vaya in meeting monthly reporting requirements required by the Department of Justice Settlement. Other duties as assigned: Work is performed with considerable independence, with most work performed in the community and away from the office. Employee is familiar with values and goals of TCL and Care Management and minimal daily instruction is required. Formal supervision is scheduled monthly on an ongoing basis and informal supervision occurs as needed. KNOWLEDGE, SKILLS, & ABILITIES Exceptional interpersonal and communication skills. Thorough knowledge of TCL requirements, waiver and contractual requirements, and knowledge of the TCLD database. Knowledge and understanding of the Diagnostic and Statistical manual of Mental Disorders (current version). Considerable knowledge of the MHSU/IDD service array provided through the network of the LME/MCO's providers. Knowledge of Medicaid eligibility determinations. Ability to independently identify, implement and assess system and organizational priorities relative to TCL Initiatives. Ability to be responsive to team members, internal LME/MCO staff, members/recipients, families, and external partners; communicate clearly and concisely through both verbal and writing skills and case formulation. Ability to make prompt independent decisions based upon relevant facts and established processes. Skill in problem solving, negotiation and conflict resolution to balance the needs of both internal and external customers. High level of diplomacy and discretion is required to effectively negotiate and resolve issues with menial assistance. Ability to independently organize multiple tasks and priorities and to effectively manage and assigned caseload under the pressure of deadlines. Skill in Microsoft Office products (such as Word, Excel, Outlook, Forms, etc.) and Vaya Information Systems is required. A high level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance. This will require exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts. Problem solving, negotiation, and conflict resolution skills are essential to balance the needs of both internal and external stakeholders. Must be highly skilled at shifting between macro and micro level planning, maintaining both the big picture and seeing that the details are covered. EDUCATION & EXPERIENCE REQUIREMENTS A minimum of a Bachelor's degree from an appropriately accredited university or college in public service administration, psychology, sociology, or social work or a human service programmatic field, preferably with coursework in human service planning and four years of experience in directly related human services program; OR Graduation from a four-year college or university and four years of administrative or consultative experience in a human service program; OR A Master's degree from an appropriately accredited university or college in a related human services field and two years of supervised clinical experience; Or equivalent combination of education and/or experience. *Direct related experience is defined a human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy, and treatment planning. AND in addition to one of the above qualifications being met, incumbent: Should have experience working with children, adolescent, and adults with MHSU and IDD diagnoses. Must have an operational knowledge of funding sources and access to existing funds or ability to develop (if needed) or present specialized funding requests. MENTAL/PHYSICAL REQUIREMENTS Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading. Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists and fingers. Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time. Mental concentration is required in all aspects of work. RESIDENCY REQUIREMENT: The person in this position is required to reside in North Carolina or within 40 miles of the North Carolina border SALARY: Depending on qualifications & experience of candidate. This position is exempt and is not eligible for overtime compensation.
