As the need for our services grows, Catholic Charities is working to adapt and respond with strategies that prevent poverty, meet basic needs in times of crisis, and create pathways to greater stability.Job SummaryThe Mental Health Case Manager I will be embedded in one of Catholic Charities' permanent supportive housing program and will be assigned to manage a case load of clients with mental health needs. The Mental Health Case Manager will report to the Program Manager with oversight from the Director of Behavioral Health. The practitioner will provide services that are rehabilitative and enable the client to develop and enhance psychiatric stability, personal and emotional adjustment, social competencies, community skills and independent living skills. The practitioner will support the client in strengthening skill areas needed according to their treatment plan. Catholic Charities provides services to clients in Hennepin or Ramsey County.Wage Range: $28.00 - $32.00/hourlyEssential FunctionsConduct individual assessments, assessing functional strengths, resources and needs.Develop individualized, person-centered, trauma informed community support plans.Implement interventions identified in support plan, support client with referrals to external providers, maintain connection with external providers and provide case coordination support.Develop therapeutic relationship with team and client. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies. Ensure confidentiality of all records and interactions.Coordinate services both internally and externally including; county case manager, family/guardian of client, work personnel, medical personnel, etc.Attend supervision, case consultation meetings and participate in team-based decisions regarding all clients and services.Maintain client files, update client files, update treatment plans a minimum of once every six months and do all required documentation as specified in DHS guidelines, utilizing professional writing skills.Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.Provide case management to support housing retention, including supporting timely rent payment, renewal of benefits, maintaining a habitable housing unit, etc.May transport clientsOther duties as assigned.Qualifications/Technical ExpertiseBachelor's in Social Work, Psychology, Sociology, Human Services, or similar fieldCompletion of a practicum or internship that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields.At least two years of supervised experience providing services to adults or children with mental illness, substance use disorder, emotional disturbance, traumatic brain injury, or developmental disabilities, plus training in mental illness recovery, mental health de-escalation, co-occurring mental illness and substance use, and psychotropic medications and side effects.Experience providing licensed mental health care directives and developing treatment plans.Valid driver's license; may be required to transport clients.Ability to work with diverse and low-income populations, including individuals experiencing homelessness, unstable housing, or mental illness.Physical RequirementsCPR - YesDriving - YesOperating specialized machinery (additional assessment may be required) - NoWalking or standing > 2 hours/shift - NoLifting up to 25lbs without assistance - NoLifting up to 50lbs without assistance - NoPhysically restraining clients - No
