Graduate of an accredited Occupational Therapy program. Current Occupational Therapist license in the state of practice. Prior SNF or geriatric rehabilitation experience preferred. Strong clinical reasoning, communication, and organizational skills. Ability to work effectively in a team-oriented environment.

Evaluate residents' functional abilities, including ADLs, mobility, cognition, and psychosocial factors. Develop and implement individualized treatment plans based on evaluation findings and physician orders. Provide direct occupational therapy services in accordance with state and federal regulations and facility policies. Monitor and document resident progress, adjusting treatment plans as necessary to meet goals. Participate in care planning meetings and collaborate with the interdisciplinary team. Educate residents, families, and caregivers on strategies to promote safety and independence. Recommend adaptive equipment and assistive devices as needed. Supervise and provide direction to Certified Occupational Therapy Assistants (COTAs) and therapy aides, as appropriate. Ensure accurate, timely, and compliant documentation in the electronic medical record.
