Medical Social Worker - Care Coordination - FT Days

Job Summary The Medical Social Worker Unlicensed collaborates with the Clinical Care Coordinator (C3) and other members of the care management and health care teams, utilizes professional Social Work skills in accordance with social work practice standards to: assist the patient in achieving optimal health; assure appropriate use of health care resources; facilitate/coordinate timely care along the continuum; decrease unnecessary variances in care through proactive care management; develop a safe and timely discharge plan in accordance with the Joint Commission and CMS Guidelines, each patient will be ensured a planned program of continuing care which meets his/her discharge needs and to ensure a smooth hospital stay with a seamless transition to the next level of care following discharge from GHS. Qualifications Masters of Social Work required Previous experience providing social work/case management services preferred Experience in a hospital environment preferred Core Competencies These competencies reflect the values and behaviors expected of all Grady team members, regardless of role.
