Transitional Services for New York, Inc. (TSINY) is seeking Case Managers for its mental health Apartment Treatment Program in Bellerose, NY, offering an annual salary of $47,320 or $26 per hour for full-time positions.

Candidates must possess a Bachelor's degree or an Associate's degree with one year of experience, or a High School diploma with two years of experience in a mental health setting.

The Case Manager will coordinate assessments, treatment planning, and care for residents, while advocating for their rights and promoting independent living skills.

Essential functions include linking residents to community resources, facilitating skill development, and ensuring integrated care with treatment providers.
