We provide patient care in San Francisco, San Mateo, Santa Clara, Contra Costa, Alameda Counties and Salona County.

What you’ll do as a Licensed Vocational Nurse (LVN) - Hospice:

Work under the direction of a RN - Case Manager
Provides skilled services to patients in the comfort of their home
Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes
Respected and treated as a vital member of the patient care team

To be a success as a Licensed Vocational Nurse (LVN) - Hospice:

1+ years Hospice Case Manager Experience required
Current and unrestricted CA RN license
Current BLS/CPR Certification
Experience working with an interdisciplinary team is highly desired
Case Management fundamentals is highly desired
Self-Directed
Critical thinker

Organized
