Responsibilities

• Evaluate patients and establish individualized treatment plans and functional goals
• Deliver therapeutic exercise, manual therapy, and gait and mobility training across a varied caseload
• Reassess progress and adjust each plan of care to keep patients advancing toward their goals
• Direct and collaborate with Physical Therapist Assistants and the interdisciplinary care team

Travel Physical Therapy Requirements:

• 2+ years of recent Physical Therapy experience.
• Must hold all required certifications and licenses as mandated by California and national regulatory bodies.

Blue Oak Post-Acute is a 181-bed skilled nursing and therapy center in downtown Santa Rosa, California, providing short-term rehabilitation and long-term care, with a behavioral-health Special Treatment Program.

• In-house physical, occupational, and speech therapy available up to seven days a week, with 24/7 skilled nursing
• Physical therapy for mobility and strength, occupational therapy for daily-living skills, and speech therapy for communication, swallowing, and cognition
• Specialty programs including cardiac recovery, a CHF program, contracture management, and dysphagia treatment
• A behavioral-health Special Treatment Program alongside the skilled nursing beds
