Speech Language Pathologist - Home Health

Our team is growing and looking for an experienced: Speech Language Pathologist Shifts Available: Full Time Service Areas with Openings: Woodlands, Montgomery, Conroe, New Caney Responsibilities Facilitates rehabilitation of patients with emotional, physical, or mental disabilities. Implements speech therapy care plans. Coordinates care with entire team. Educates patients and family members. Prepares timely, complete, and accurate clinical notes. Communicates effectively with patients, caregivers, healthcare providers, office support and other care team members. Qualifications Speech language pathologist in the State of Texas. Home health experience highly preferred. Experience with Home Care Home Base EMR (preferred but not required). Knowledge of home health regulations and general healthcare compliance. Ability to handle physical workload. Good driving record. Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook). Exceptional communication and people skills.
