Registered Nurse Home Health

Job Summary:

Provides comprehensive home health nursing care, coordinating patient visits based on physician orders and agency protocols. Responsible for supervising home health aides and LPNs/LVNs, ensuring quality care delivery in patients homes.

Responsibilities:

• 
  Coordinate and deliver patient care through the application of nursing processes during home visits.
  
  
• 
  Supervise and evaluate the performance of home health aides and licensed practical nurses/licensed vocational nurses.
  
  
• 
  Document patient assessments, treatments, and progress accurately within required timeframes.
  
  
• 
  Travel up to 75 miles from the primary care center to conduct patient visits in homes and occasional assisted living facilities.
  
  
• 
  Comply with agency policies, state regulations, and documentation standards, including proficiency in HomeCare HomeBase and Oasis.
  
  
• 
  Participate in periodic office meetings and on-call rotations as scheduled.
  
  

Qualifications:

• 
  Valid license to practice as a Registered Nurse in the state of assignment.
  
  
• 
  Minimum of 1 full year of recent adult home health or hospice experience.
  
  
• 
  Proficient in Oasis documentation and experienced with HomeCare HomeBase EMR.
  
  
• 
  Reliable transportation, valid drivers license, and auto insurance.
  
  
• 
  Ability to travel up to 75 miles and manage a caseload of 5-8 patients daily.
  
  
• 
  Willing to accept on-call responsibilities and work a flexible schedule if required.
