The position does involve direct patient care for a population of patients ages 18 and older. Age specific experience and/or special training and/or expertise is required to serve this population.

The Family Practice Nurse Practitioner provides comprehensive primary care services to patients of all ages at Coalinga Regional Medical Center Family Practice Clinic, in compliance with the California Nurse Practice Act and in collaboration with the supervising physician. The NP is responsible for health promotion, disease prevention, diagnosis, and management of acute and chronic conditions.

Position Qualifications:

• Must have completed an accredited Nurse Practitioner Program.
• Current nursing licensure in State.
• Experience in family practice, primary care, or a related outpatient clinical setting preferred.
• Maintains controlled substances registration with state and federal authorities, if applicable.
• Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.
• Not excluded from Medicare or Medi-Cal programs. Must maintain DEA registration for prescribing controlled substances as applicable.
• Must possess and maintain an active Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross at all times.
• The ability to communicate well, both verbally, and in writing.

Duties and Responsibilities:

• Providing comprehensive primary care services to patients of all ages including well visits, acute illness, and chronic disease management.
• Conducting thorough patient history and physical examinations and documenting findings accurately in the electronic health record.
• Ordering and interpreting diagnostic tests including laboratory and imaging studies and developing appropriate treatment plans.
• Prescribing medications and treatments in accordance with California law, CRMC protocols, and evidence-based practice guidelines.
• Collaborating with physicians, specialists, and other members of the healthcare team to coordinate comprehensive patient care.
• Providing patient and family education regarding diagnosis, treatment plans, medications, and preventive health measures.
• Managing and monitoring patients with chronic conditions such as diabetes, hypertension, asthma, and heart disease in accordance with current clinical guidelines.
• Reviewing and responding to patient results, referrals, and communications in a timely manner to ensure continuity of care.
• Providing after-hours on-call coverage as assigned and ensuring continuity of patient care.
• Participating in quality improvement initiatives, peer review, and departmental meetings as required.
• Developing and implementing individualized patient care plans in collaboration with the supervising physician and interdisciplinary team.
• Maintaining accurate, timely, and complete documentation of all patient encounters in the electronic health record in compliance with CRMC and regulatory standards.
• Maintaining current knowledge of evidence-based clinical guidelines, research, and trends in family practice and primary care.
• Working collaboratively with the clinic team including physicians, medical assistants, front desk staff, and other clinical personnel.
• Participating in performance improvement programs, quality audits, and patient satisfaction initiatives as directed.
• Reviewing and contributing to the development of clinical protocols and policies to ensure the highest standard of patient care at CRMC.
• Providing culturally sensitive care and effective communication with patients and families from diverse backgrounds.
• Adhering to all CRMC policies, HIPAA regulations, and California licensing requirements at all times.

Salary Description $124,800 - $150,000 Yearly
