New team members receive a structured orientation with a nurse preceptor, and the organization supports continuing education for nurses who want to advance, including pathways from LPN to RN for those interested in further growth.Core ResponsibilitiesAdminister medications and treatments accurately according to physician orders and facility policyComplete resident assessments, monitor changes in condition, and report concerns to the RN supervisor and providerPerform wound care, catheter care, injections, and other skilled nursing procedures within LPN scopeSupervise and support CNAs on the assigned unit to ensure care is delivered safely and on timeDocument assessments, medication administration, and care in the electronic health recordParticipate in care planning, admission, and discharge processesRespond to emergencies and initiate appropriate interventionsCommunicate clearly with residents, families, therapy, and the interdisciplinary teamFollow infection control, fall prevention, and resident rights standardsTake part in shift reports, audits, and ongoing clinical educationMentor newer CNAs and nurses and model professional, resident-centered careMaintain medication carts, supplies, and treatment records in compliance with regulationsRequired QualificationsActive, unrestricted LPN license in the state of practice or an eligible compact licenseCurrent BLS or CPR certificationOne year or more of long-term care, skilled nursing, or rehab experience preferred; new graduates are welcome to applyWorking knowledge of electronic health records and medication administration systemsStrong clinical judgment, organization, and communication skillsAbility to pass a background check and health screeningReliable attendance and flexibility to work some weekends and holidays as scheduledCommitment to resident dignity, privacy, and person-centered careBenefits & PayPay: $35.00 - $45.00 per hour, based on experience and schedule.
