Job Summary We seek a highly skilled and experienced Registered Nurse (RN) to provide skilled, patient-centered home health care in Mesa County, CO. The RN will perform comprehensive patient assessments, develop individualized care plans, provide skilled nursing interventions, monitor patient progress, and collaborate with interdisciplinary team members. The role requires strong communication, critical thinking, and the ability to work independently managing a structured caseload, with participation in an on-call rotation. Responsibilities Perform comprehensive patient assessments in home health settings. Develop and update individualized plans of care for patients. Provide skilled nursing care and appropriate interventions. Monitor patient progress and report changes to the care team. Educate patients and families on medications, treatments, and care plans. Coordinate care with LPNs, therapists, caregivers, and healthcare providers. Complete timely and accurate documentation in electronic medical records (EMR). Participate in an on-call rotation to support patient needs. Required Skills & Certifications Active Registered Nurse (RN) license in the state of Colorado or active Nurse Licensure Compact (NLC) license. Valid driver's license, reliable transportation, and current auto insurance. Strong clinical assessment and critical thinking skills. Ability to work independently and manage a structured caseload. Clear and professional communication skills with patients and team members. Proficiency with documentation and EMR systems (experience with HCHB preferred). Preferred Skills & Certifications Previous home health nursing experience. Bachelor's degree in Nursing or related field. Special Considerations Position includes participation in an on-call rotation. Work is performed in patients' homes, requiring travel within Mesa County. Ability to adapt to varying home environments and patient conditions. Scheduling Full-time position with Monday through Friday day schedule.
