Work Environment\n

This position operates in a fully remote setting, providing flexibility for professionals who are skilled in delivering social work services from a distance. While you will work independently, you will also play an integral role in a collaborative team comprised of healthcare professionals dedicated to serving individuals and families in hospice care.

Key Responsibilities\n

• Conduct thorough evaluations and assessments of clients within the hospice framework to understand their emotional and psychosocial needs.

• Utilize your expertise to develop and implement personalized care plans in collaboration with clients and their families.
\n
• Facilitate Individualized Family Service Plan (IFSP) meetings, guiding families in navigating the complexities of hospice services.
\n
• Regularly monitor client outcomes, making necessary adjustments to care plans based on observed progress and family input.
\n
• Offer educational support and resources to families, helping them develop coping mechanisms to assist in the grieving process.
\n
• Engage in team meetings and interdisciplinary discussions to promote a holistic approach to client care.

• Maintain compliance with all Federal and State regulations pertaining to social work practices.
\n
• Utilize evidence-based practices to foster a nurturing and supportive environment for clients and their families.

• Adhere to ethical standards and confidentiality requirements to assure clients’ rights and safety are respected.

Patient Population / Client Focus\n

The main focus of this role is individuals and families in hospice care, often requiring specialized approaches to address end-of-life issues. This includes working with clients suffering from terminal illnesses and their families, ensuring they receive not just physical care but also emotional and psychological support tailored to their unique situations.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.
\n
• Current licensure as a Licensed Clinical Social Worker (LCSW) from the Department of Public Health.
\n
• One year of direct experience in social work is required, preferably within hospice or palliative care environments.
\n
• Valid driver's license is preferred to facilitate client engagement if in-person visits are necessary.
\n
• Proficient in various learning styles and able to adapt treatment methodologies to meet individual client needs.

• Strong organizational, time management, and communication skills are essential to succeed in this role.

Compensation and Benefits\n

This position offers a competitive salary range of USD 75,000 to 80,000 per year, commensurate with experience. Additional benefits include:
