In this role, you will primarily create and manage a care plan for individual patients. You are a Vocational Nurse with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and can observe and communicate effectively in your patients' best interests. You should have demonstrated knowledge and skill in current nursing practices and a strong penchant for solving problems to succeed. ResponsibilitiesCoordinate with colleagues to assess, implement, or evaluate patient care plans.Administer medications and monitor patients for side effects or reactions Document any changes in patient conditions and report on changesMaintain accurate records of patient care and discuss observations with the supervisor QualificationsCurrently Licensed Vocational Nurse(LVN) in the state of employmentPrevious experience as a LVN is preferredExcellent interpersonal and client care skillsValid driver’s licenseFirst aid/CPR certification required
